Vilken sammanställd forskning finns om behandlingseffekter av Fokuserad Acceptance and Commitment Therapy vid depression och ångestsyndrom?

Sammanfattning

SBU:s upplysningstjänst har efter litteratursökning inte identifierat någon relevant systematisk översikt som undersökt effekten av Fokuserad Acceptance and Commitment Therapy (FACT) jämfört med traditionell kognitiv beteendeterapi (KBT), ACT eller annan aktiv terapibehandling.

Bakgrund

Depression och ångest ses idag som folksjukdomar som kan drabba människor i alla åldrar. Uppskattningsvis kommer var tredje kvinna och var fjärde man i Sverige någon gång i livet att utveckla en depression, och ångestsyndrom förekommer hos ungefär var fjärde person i hela befolkningen [1]. Ångestsyndrom omfattar flera diagnoser, bland annat generaliserat ångestsyndrom, paniksyndrom och specifika fobier. Vid de flesta former av lindrig och medelsvår depression eller ångestsyndrom rekommenderas psykologisk behandling, i första hand KBT [1].

ACT är en vidareutveckling av KBT, en så kallad tredje vågens KBT, som används vid både somatiska sjukdomar, såsom kronisk smärta, och flera olika psykiatriska tillstånd, däribland depression och ångestsyndrom. Fokuserad ACT (FACT) är en terapimetod som baseras på ACT men med ett mindre omfattande program. Medan en ACT-behandling ofta består av mellan 8 och 20 sessioner, består FACT vanligtvis av mellan en och fem sessioner. Metoderna baseras på samma centrala processer, acceptans och medveten närvaro (mindfulness), vilka är kopplade till det övergripande målet psykologisk flexibilitet. Psykologisk flexibilitet handlar bland annat om att vara i kontakt med nuet och att välja beteenden som leder till värdefulla mål i livet för individen [2]. Behandlingarna ges framför allt av legitimerade yrkesgrupper såsom psykologer, psykoterapeuter och läkare.

Psykisk ohälsa är en vanlig orsak till besök inom primärvården [3]. För att möta behovet av psykologisk behandling finns ett intresse av att veta om FACT, som är ett mindre omfattande program än ACT, skulle kunna utgöra en alternativ metod för primärvården.

Avgränsningar

Upplysningstjänsten har gjort sökningar (Bilaga 1) i databaserna Medline (Ovid), Scopus och PsycInfo (EBSCO). Vi har även sökt publikationer på webbsidor för HTA-organisationer.

Upplysningstjänsten har tillsammans med frågeställaren formulerat frågan enligt följande PICO1:

1. PICO är en förkortning för patient/population/problem, intervention/index test, comparison/control (jämförelseintervention) och outcome (utfallsmått).

Litteratursökningen har begränsats till systematiska översikter.

För att vi skulle inkludera en artikel i svaret krävde vi att den var publicerad på engelska eller ett av de skandinaviska språken och att den hade genomgått en peer review.

Resultat från sökningen

Upplysningstjänstens litteratursökning genererade totalt 764 artikelsammanfattningar (abstrakt) efter dubblettkontroll. Ett flödesschema för urvalsprocessen visas i Bilaga 2. Två utredare på SBU läste alla artikelsammanfattningar och bedömde att 59 översikter kunde vara relevanta för frågan. Dessa artiklar lästes i fulltext av två utredare och de artiklar som inte var relevanta för frågan exkluderades. Exkluderade artiklar finns listade i Bilaga 3.

Upplysningstjänsten fann inte någon systematisk översikt som undersökt FACT jämfört med traditionell KBT eller någon annan terapimetod. Då Upplysningstjänsten inte fann någon systematisk översikt som var relevant för denna fråga redovisas därför inga resultat eller slutsatser i Upplysningstjänstens svar.
